HEALTH INSURANCE NON-COMPLIANCE PENALTY
Residential students registered for 5 or more credits for any semester who, by August 15, have not submitted with a waiver request form* or enrollment form* for health insurance will be assessed the $1,000 non-compliance penalty fee.
If a waiver request form* or enrollment form* is received after the non-compliance penalty fee is assessed, the penalty fee will be pro-rated on a daily basis (from date penalty fee is assessed until date from is received).
The non-compliance penalty fee is only eligible to be refunded during the semester in which it is charged. No pro-rated refunds will be issued after the last day of class of the semester in which the non-compliance penalty fee is charged.
The non-compliance penalty fee will also be pro-rated on a daily basis for any student approved for a medical leave of absence (from date penalty fee is assessed until date of withdrawal).
If at any point during the academic year a student is without health insurance coverage, the student will be assessed the non-compliance penalty fee for the period of time they are without coverage up to $1,000.
